Affiliate Transfer Request

This is the Transfer Request Form to streamline the process of assigning a customer's purchase to you as their affiliate for the Rewired Course launch. To complete the transfer, you must submit this form along with any supporting documentation that confirms the customer wants to be associated with your affiliate account.

Transfers will only be approved with clear proof that the customer personally requested the change. The most reliable option is for the customer to forward their purchase receipt to you. You can also include screenshots of written communication (emails, DMs, etc.) where they directly ask to be connected to your affiliate account.

Because many affiliates have overlapping audiences (and customers often choose based on the bonus package an affiliate offers) we require direct confirmation from the customer to ensure their choice is honored and the process remains fair for everyone.

*All requests should be submitted no later than Monday, November 10th, 2025*

Do You Confirm That The Customer Initiated This Request?

✅ Yes, the customer personally asked to be connected to me/ my bonus package

❌ No, the customer did not directly ask to be transferred (Transfers will not be approved without customer confirmation)

🟡 I’m not sure, this is a unique situation and I need to speak with the affiliate manager before proceeding (if so, please do not complete this form and email [email protected] directly)

Do you have proof of the customer’s request ready to upload now?

✅ Yes, I have the customer’s receipt or a written message from them

❌ No, I’ll return and complete this form once I have the required proof
